Once you've decided on the PACC/E-ZPass program and payment option that's best for you, simply fill out the application and return it to us. You can pick up an application at any of the toll plazas along the West Virginia Turnpike or you can call our Customer Service Center at 1-800-206-6222 and request one to be sent to you. If you would like to pick up the transponder, you may stop by our Customer Service Center, Monday through Friday 8:00 A.M. to 4:30 P.M. at 3310 Piedmont Road, Charleston, West Virginia 25306 and fill out the application and leave with the transponder.   You can also download the application below and mail, fax or stop by our office and complete the process.
